Applying to Middle School [Grades 6 - 8]
We are delighted that you’ve chosen to apply to Mid-Pacific Institute. There are nuances in the application process at different grade levels. We’ve tried to make them easy to follow for you and your child. At any time you have questions, do not hesitate to contact us.
Priority Application Deadlines
Grades 6-12: Postmarked by December 1, 2008.
Teacher references and supplemental information due by January 31, 2009.

- Submit completed application to the Office of Admissions with the $75 Application Fee (non-refundable). See below for mailing address.
- After December 1, 2008, provide TWO teachers with the Teacher Reference Report.
- Download the Teacher Reference Report forms below.
Grades 2-12 HAIS Teacher Reference Report 
- Sign the Teacher Reference Report before forwarding it to your teacher.
- Provide the teacher with a stamped envelope addressed to:

- Forms must be received directly from teachers by January 31, 2009.
- Applicants to grade 6-8: Submit copies of report cards and standardized test results from 4th grade through the first semester of the current school year.

Required Testing
- Apply for the appropriate required tests. Have the results forwarded to Mid-Pacific by the test centers.
- Applicants to grades 6-11: SSAT (Secondary School Admissions Test), MPI Code:5080
- English as a Second Language applicants for grades 7-9: SLEP (Secondary Language English Proficiency test)
- Can be taken on campus by appointment.
- $100 US test fee includes the SLEP, an interview and writing sample.
